    Attempted prison escape in North Carolina leaves two dead, multiple injured

    An attempted escape from a North Carolina prison left two people dead and multiple others injured Thursday, authorities said.

    The incident occurred at the Pasquotank Correctional Institute in Elizabeth City, where fires were set in the prison's sewing plant around 3:30 p.m., the North Carolina Department of Public Safety said in a tweet Thursday afternoon.

    Local media reported, citing Pasquotank County Sheriff Randy Cartwright, that two employees had died following the attempted prison break. Officials didn't say whether the victims were employees or inmates.

    Authorities said 14 people had been injured.

    The Elizabeth City-Pasquotank Public Schools said several area schools had been placed on lockdown.

    The public safety department said several employees were among the injured. The extent of their injuries was not immediately clear.

    The fires have been extinguished and the prison is placed under lockdown, the department said, adding that no inmates have escaped.

    Sentara Albemarle Medical Center in Elizabeth City took in nine people, hospital spokesman Peter Sengenberger said.

    Five people were transported to another hospital in Norfolk, Va., the spokesman said.

    “Due to an incident that is taking place at the local jail north of town, Northside Elementary, Elizabeth City Middle School and Pasquotank County High School have been placed on lock down at the advice of Emergency

    Management,” the district said in a Facebook post.

    The district had later lifted the lockdown, and students were allowed to go home.

    The high-security all male prison holds 896 inmates, and has a staff size of 410 employees, according to the prison's website. However, the Department of Public Safety said it houses 729.

    UPDATE: NCDPS identifies two employees killed at Pasquotank Correctional Institution

    PASQUOTANK COUNTY, NC (WITN) - The North Carolina Department of Public Safety has identified the two prison employees who died during an attempted prison break Thursday afternoon at Pasquotank Correctional Institution.

    Correctional Officer Justin Smith, 35, provided security in the Correction Enterprises Specialty Sewing Plant. He had worked as a correctional officer since 2012.

    Correction Enterprises Manager Veronica Darden, 50, supervised inmates working in the Specialty Sewing Plant. She had been a Correction Enterprises employee since 2007 and previously worked as a correctional officer.

    Three other prison employees remain hospitalized in critical condition at Sentara Norfolk General Hospital. Seven prison employees were treated and released from Sentara Albemarle Medical Center in Elizabeth City. Four inmates were treated for injuries.

    One injured prison worker no longer critical; sheriff reveals details of incident

    The condition of one of three prison workers critically injured in Thursday’s attempted prison break in Pasquotank County is apparently improving. Meanwhile, seven other prison employees being treated for injuries they suffered in the incident have been released.

    A press release from Norfolk-based Sentara Healthcare on Friday said two of three patients from Thursday’s attempted prison break in Pasquotank County are still in critical condition at Sentara Norfolk General Hospital. A third is in fair condition, the release said.

    Sentara’s release said Sentara Albemarle Medical Center in Elizabeth City received a total of 14 patients from the incident at Pasquotank Correctional Institution. Three were transferred to the Norfolk hospital’s Level I trauma center Thursday evening.

    Prison officials said early Friday that 10 of those patients were prison workers and four were inmates. All 10 apparently have been released from the hospital.

    Two prison workers, a Corrections Enterprises manager and a correctional officer, were killed in Thursday’s attempted inmate escape. Prison officials have identified them as Veronica Darden, 50, and Justin Smith, 35.

    Pasquotank Sheriff Randy Cartwright said the condition of one of the employees had improved enough to allow investigators to conduct an interview.

    "So, that is a lot better news than I had early this morning," he said.

    Cartwright also released more details about the attempted escape at the prison on Thursday.

    Cartwright said investigators believe four inmates planned their breakout by starting a fire inside a sewing plant to divert guards, then running through a loading dock to reach the fence. The plot was foiled and all inmates have since been accounted for.

    Cartwright says stabbing or slashing wounds predominated among the injuries to correctional staff. He says some people suffered smoke inhalation, but the fire didn’t cause serious burns. The sheriff says none of the injured suffered gunshot wounds, but wouldn’t say whether guards fired on inmates to stop the attempted breakout.

    Cartwright said there is video footage of the incident at the prison.

    "We downloaded, as part of our investigation, several different videos from out there," he said, referring to the prison, adding that he hasn't personally seen the video. "We're not going to have this whole incident from start to finish on video, but we're going to have a large portion of it from what I saw this morning before I left (the prison area) a little after 5 (a.m.)"

    Cartwright said the SBI has sent an arson investigation team to probe the fire. Asked how the fire began, he said he has “some inkling” but declined to share it.

    Cooper directs prisons to improve security after fatal attack

    State officials have suspended the inmate work program at the Pasquotank Correctional Center in Elizabeth City prison and ordered more officers in areas where inmates work.

    The orders follow a fire and escape attempt Thursday that left two prison employees dead and 10 inhured.

    On Friday, Public Safety Secretary Erik A. Hooks and state prison leaders met with Governor Roy Cooper, who directed them to improve prison safety.

    “We owe it to the public and to our employees to keep state prisons secure, and we owe it to these fallen employees and their families to learn all we can about what happened and take steps to prevent a similar tragedy from happening again,” Hooks said in a statement.

    The statement listed steps it said the department was taking in an effort to improve safety:

    Suspending Correction Enterprises operations at Pasquotank Correctional Institution while a thorough review of the program’s safety and security is completed.

    Reviewing the safety of all Corrections Enterprises operations at all state prisons.

    Increasing the number of correctional officers who provide security in Correction Enterprises areas, and increasing the number of rounds by the officer in charge within those areas until such time as we are able to conduct a security review to ensure the safety of employees.

    Reviewing emergency procedures at all facilities to enhance safety and security. Conducting a complete facility search at Pasquotank Correctional Institution by 100 trained and well-equipped Prison Emergency Response Team members. The search will look for evidence as well as unrelated contraband that may be in the facility.

    Correction Enterprises employs about 2,500 inmates statewide, teaching job skills in 17 different industries, while producing goods and providing services for government agencies.

    Around 3 p.m. Thursday, inmates started a fire in the prison’s specialty sewing plant, where about 30 inmates work producing embroidered logo items, safety vests and other sewn items. The fire was extinguished. After the fire was set, several inmates tried to escape but were not successful.

    Correctional Officer Justin Smith and Correction Enterprises Manager Veronica Darden were killed by inmates in the escape attempt. Ten other prison employees were injured. Correctional Officers Wendy Shannon, Correctional Officer George Midgett, and maintenance worker Geoffrey Howe remain hospitalized for their injuries.

    Pasquotank Sheriff Randy Cartwright reiterated Friday that no prisoners had escaped, in spite of numerous reports to the contrary on social media.

    “We found no evidence that anybody left the yard,” Cartwright said. “This was not an escape.”

    Cartwright also said the attempted escape wasn’t spontaneous, and several crime scenes throughout the prison were being processed. Numerous items that could be used as weapons were also discovered, he added.

    The deadly fracas was confined to the sewing plant, Cartwright said, adding that the incident couldn’t be defined as a riot. The fire in the sewing plant was set, and as it was being put out, four inmates tried to escape, making it only as far as the yard of the sewing plant.

    The four inmates believed to have been responsible for the escape attempt were also taken to Sentara Albemarle, and later transported to Polk Correctional Institute in Butner, where they were being held in the facility’s most secure area, Cartwright said.

    The Pasquotank County Sheriff’s Department and the State Bureau of Investigation are conducting a criminal investigation into the attack and attempted escape.

    Correctional Officer Wendy Shannon and maintenance worker Geoffrey Howe remain hospitalized, according to the N.C. Department of Public Safety.

    Correctional Officer Justin Smith, 35, and Correction Enterprises manager Veronica Darden, 50, were killed by inmates in the escape attempt. Ten other prison employees were treated and released.

    The attempted breakout began around 3 p.m. when inmates set a fire in the prison’s sewing plant, where about 30 inmates produce embroidered logo items, safety vests and other items for government agencies and nonprofits, the state Department of Public Safety said.

    Several inmates tried to escape as the fire was being extinguished, but they were unsuccessful, officials said.

    Inmates beat prison employees with hammers and stabbed them with scissors, according to one prison worker who called 911.

    Pasquotank Correctional Institution, which houses about 720 male inmates in close, medium and minimum custody, remains on lockdown.

    4 inmates charged with murder after attempted escape at Pasquotank prison

    Elizabeth City N.C. - Four inmates already serving lengthy sentences for violent crimes have been charged with two counts of first-degree murder each in the deaths of two prison employees in Pasquotank County during an attempted escape last week.

    Wisezah Buckman, 29; Seth Frazier, 33; Mikel Brady, 28; and Jonathan Monk, 30, were charged Thursday in the deaths of Correctional Officer Justin Smith and correction enterprises manager Veronica Darden.

    “We intend to prosecute to the fullest extent,” District Attorney Andrew Womble said during a Friday news conference announcing the charges.

    The four men could face the death penalty or life in prison without parole in convicted, he said.

    Buckman was serving 32 years and three months for a 2015 second-degree murder conviction. He also had been convicted of other violent crimes including assault with a deadly weapon with intent to kill and armed robbery.

    Frazier was jailed in 2012 for burglary, and was to serve just under 11 years. He also had been found guilty of possession of a firearm by a felon, indecent liberties with a child and resisting an officer among, other crimes.

    Brady was convicted in 2014 for attempted first-degree murder and sentenced to 24½ years. Other crimes on his record include assault with a deadly weapon with intent to kill and assault of a law enforcement officer with a firearm.

    Monk was serving 16½ years after a 2014 conviction for attempted first-degree murder. Also on his record are convictions for assault with a deadly weapon with intent to kill and burglary.

    The inmates are accused of setting a fire before attempting to escape from the sewing plant of the Pasquotank Correctional Institution on Oct. 12. Ten other prison employees were injured. Two remain hospitalized in critical condition at last report.

    Police responded within minutes and arrested four inmates before they could scale the two razor-wire-topped fences surrounding the prison located west of Elizabeth City.

    “This is the most serious matter I’ve ever prosecuted,” Womble said at a press conference held Friday in Elizabeth City.

    The investigation continues and more charges are pending, he said. Police did not release the weapons used or details of the attempted escape.

    It was the deadliest escape attempt ever for the North Carolina prison system. The state is reviewing safety practices in corrections work programs, including their use of violent offenders.

    The program teaches work skills to 2,500 inmates in 17 industries in prisons across the state. About 30 inmates worked in the sewing plant at Pasquotank, where they produced embroidered logos, safety vests and other sewn items. Officials shut down the plant after the attempted escape.

    ELIZABETH CITY, N.C. (WNCN) – 911 calls released Monday reveal the moments following an attempted escape from the Pasquotank Correctional Institution on Oct. 12 that left two employees dead and multiple others injured.

    Around 3 p.m., inmates started a fire in the prison’s Correction Enterprises Specialty Sewing Plant, the North Carolina Department of Public Safety said. At that point four inmates attempted to escape.

    During that attempted escape, several prison employees were assaulted, and two were killed.

    “We got an officer down over here,” one caller told dispatch in a brief conversation.

    Another caller had locked herself in a closet but could still see the body of an officer on the ground.

    “The officer is alive but he is not getting up,” the caller said. “I don’t know if he was attacked with a hammer or a screwdriver.”

    A third 911 recording revealed the caller reported five inmates attempting to escape.

    “We got inmates trying to jump over the fence. Five inmates,” the caller said. Officials later confirmed four inmates attempted to escape but none were successful.

    That same 911 caller reported more officers were hurt.

    “Three officers down that we know of,” they told dispatch. “We think they were beaten a hammer but we’re not sure.”

    Many of the people who called 911 weren’t fully aware of what exactly was occurring at that time at the prison.

    The Department identified the two employees who died as Correctional Officer Justin Smith, 35, and Correction Enterprises Manager Veronica Darden, 50.

    The inmates charged with killing Smith and Darden are Wisezah Buckman, Seth J. Frasier, Mikel Brady and Jonathan M. Monk.

    Each is charged with two counts of first-degree murder. The four inmates have all been transferred to other prisons.

    Third officer dies Monday after Oct. 12 attempted escape from Pasquotank prison

    ELIZABETH CITY, N.C. - A third correctional officer died Monday evening from injuries she received when four inmates attempted to escape Oct. 12 from Pasquotank Correctional Institution.

    Wendy Shannon, 49, died at Sentara Norfolk General Hospital, according to a release from the North Carolina Department of Public Safety. She had been a correctional officer for four years.

    “Our hearts are broken at the passing of Officer Shannon,” said Department of Public Safety Secretary Erik A. Hooks. “Our deepest condolences go out to the Shannon family during this difficult time.”

    Gov. Roy Cooper also expressed condolences in a statement.

    “Our hearts go out to the victim, her family, friends and coworkers,” he said.

    Correctional officer Justin Smith and corrections enterprises manager Veronica Darden died the day of the escape. A fourth prison employee, mechanic Geoffrey Howe was in critical condition at last report.

    Wisezah Buckman, 29; Seth Frazier, 33; Mikel Brady, 28; and Jonathan Monk, 30 are charged with first degree murder and face the death penalty or life in prison without parole if convicted.

    The Pasquotank prison remains on lock down, the release said, and the sewing plant is permanently closed as officials review safety standards following the deadliest escape attempt in the history of the state’s prison system.

    The four inmates with violent crimes in their past started a fire in the sewing plant to create a diversion before breaking from the facility toward the exterior of the prison building. The sewing plant allowed about 30 prisoners access to tools such as scissors and hammers.

    The inmates were captured at gun point as they tried to breach the prison fences topped with razor wire.

    NORFOLK - A fourth prison employee died Thursday from injuries sustained during an Oct. 12 inmate escape attempt at the Pasquotank Correctional Institution in Elizabeth City, N.C., according to the North Carolina Department of Public Safety.

    Mechanic Geoffrey Howe had been in critical condition at Sentara Norfolk General Hospital since the escape attempt.

    “Our thoughts, prayers and condolences go out to the Howe family,” state Department of Public Safety Secretary Erik A. Hooks said in a statement late Thursday. “This tragedy has horribly impacted the lives of four families, as well as co-workers and friends.”

    Howe was 31 and celebrated his one-year anniversary with the department in August.

    On Monday, correctional officer Wendy Shannon died from her wounds.

    Correctional officer Justin Smith and Correction Enterprises manager Veronica Darden also died from injuries during the escape attempt, which is the deadliest in North Carolina history.

    Four inmates have been charged with first-degree murder.

    The inmates are accused of setting a fire before attempting to escape from the sewing plant at the Pasquotank County facility.

    Ten prison employees were injured.

    The prison houses about 676 male inmates.

    This is awful. They need to seriously look at who they allow to participate in these work programs and increase the number of guards who are overseeing them.
